FreeStyle Libre 2 Plus: Eligibility and Application Guide

Who is eligible? 

Eligibility to CGM technology remains unchanged. People living with type 1 diabetes and rare conditions like type 1 diabetes on the National Diabetes Services Scheme (NDSS) are eligible to receive CGM technology at a subsidised rate.  

How to apply 

To benefit from this subsidy and to update your access to the FreeStyle Libre 2 Plus, people must meet with authorised diabetes health professionals.  

Health professionals authorised to update your access:  

  • Credentialled Diabetes Educators (CDEs) 
  • Diabetes Nurse Practitioners 
  • Endocrinologists 

Once access is updated, the sensors can be ordered from NDSS Access Points, typically community pharmacies. 

Important: General practitioners and practice nurses are not authorised to certify these forms. Diabetes health practitioners must be engaged. If you do not have access to an authorised health professional, our Credentialled Diabetes Educators at the Diabetes Victoria Clinic can update your access. Life Span 

Each FreeStyle Libre 2 Plus sensor has a lifespan of 15 days, and individuals can order up to three months worth of sensors per transaction. The co-payment for each sensor is set at $17.35. It is important to understand the lifespan of your device and allocation limits, as well as what to do should your allocation be exhausted. 

Note: The existing FreeStyle Libre 2 Reader is compatible with the new sensors, so there's no need for a new reader if you already have one. 

Supply management 

It is important for those planning to switch to the FreeStyle Libre 2 Plus to ensure they have enough of their current CGM supplies to last until mid-April 2025. This precaution is to avoid any potential delays during the initial launch period. 

Alternative Glucose Monitoring 

In the event of limited supply where the FreeStyle Libre 2 Plus CGM becomes unavailable, it's important to use other blood glucose testing methods, such as a blood glucose meter and test strips. 

These tools are needed for effectively monitoring and managing your blood glucose levels. If you need help or have any questions about using these methods, you should schedule a consultation with your health professional. 

They can offer support to ensure you are using the equipment correctly and getting the most out of your readings.
