Terms and Conditions
Payment for the 8 week group program is paid upfront after your pre-group appointment. For those with a Referral for Group Allied Health Services under Medicare for patients with type 2 diabetes, Medicare rebates ($19.30) will be processed after attending the group appointments each week.
Refunds will not be issued for missed appointments. However, if you are unable to complete any of the 8 group sessions, you will be offered one on one time with the Diabetes Victoria Clinic Exercise Physiologist or to be rebooked for a group session at a later time. Please note however that if you rebook for a follow up one on one appointment, Medicare rebates under the Referral for Group Allied Health Services under Medicare for patients with type 2 diabetes cannot be processed.
Those without a Medicare Allied Health group referral (e.g. persons type 1 diabetes) will still be required to get a Medical Clearance from your GP.
If you have private health insurance, you may be covered for ‘dietetics and nutrition’ or ‘psychologist’ through your extras cover.
Please check with your provider.
As well as private health insurance, you may be eligible for assistance from:
- Medicare
- WorkCover
- TAC (Transport Accident Commission)
- A compensation fund.
- National Disability Insurance Scheme (NDIS) – We can see people on ‘Plan-managed’ and ‘Self-managed’ levels of the NDIS.
- We welcome DVA clients, DVA Health Cards (Gold and White) are accepted as payment upon a GP referral.
